How insurance works here
Your insurance policy is a contract between you and your insurance company. As a courtesy the office will submit a claim on your behalf, but because that is never a guarantee of payment, payment is made at the time of service.
Some, and perhaps all, of the services may not be covered by your plan. It is worth calling your insurance company before your visit to ask.
- Everyone completes the Information and Insurance Intake Form before seeing the doctor.
- Minors: the accompanying adult or parent is responsible for payment.
- Missed appointments without 24 hours’ notice are charged as a normal office visit.
